- Review Action Lists weekly to identify and address new flags promptly
- Update patient records during or immediately after clinic visits
- Encourage patients to use MyCBDR for real-time treatment logging
- Document all patient contacts (phone, email, clinic visits) in CBDR
- Export and archive Action List reports for quality improvement tracking
- For novel therapy patients, use the dedicated Gene Therapy App for patient-reported data
Data Entry Priorities
High priority (enter within 24 hours):
- Adverse events (especially for novel therapies)
- Gene therapy infusion date and product
- Mortality records
Standard priority (enter within 1 week):
- Clinic visit documentation
- Treatment plan changes
- Laboratory results
- HJHS scores
