This Action List flags hemophilia patients who do not have an active treatment plan recorded in CBDR. It also indicates whether the patient has any product usage recorded despite having no treatment plan.

Remediation Steps

  1. Review patient chart: Check the clinic chart or EMR for the current treatment regimen.
  2. Identify regimen type: Determine if the patient is on prophylaxis, on-demand treatment, or a non-factor therapy (e.g., emicizumab).
  3. Create treatment plan in CBDR: Navigate to the Treatment Plan module and enter the prescribed regimen including product, dose, and frequency.
  4. Set effective date: Backdate the plan effective date to when the regimen was actually initiated if known.
  5. Reconcile usage records: If usage exists without a plan, verify the products used align with the newly created plan.
