Remediation Steps

  1. Verify patient status: Contact the patient or their caregiver to confirm current status and care arrangements.
  2. Update mortality status: If the patient has passed away, record the mortality date in CBDR and complete the mortality adverse event form.
  3. Transfer records: If the patient has transferred to another HTC, update the primary HTC assignment in CBDR.
  4. Update active status: If the patient is no longer receiving care, update their status to inactive with appropriate documentation.
  5. Document contact attempt: Record the phone consultation or clinic review in CBDR to update the last activity date.
